Today, in vitro fertilization (IVF) is practically a household word. But not so long ago it was a mysterious procedure that produced what were then known as "test-tube babies." Louise Brown, born in England in 1978, was the first such baby to be conceived outside her mother's womb.
The fluid from the follicles is examined under the microscope by the embryologist, who locates the eggs and keeps them in the laboratory under physiologic conditions. The embryologist will place the sperm with the eggs when they are ready for fertilization. Usually, the eggs will develop into cleaving pre-embryos, whose cells divide 2 or 3 times to become preimplantation embryos (pre-embryos). They are maintained in laboratory dishes, in a nutrient mixture which acts as a substitute for the environment that would otherwise have been provided by the fallopian tubes.
In vitro fertilization (IVF) is a technique in which egg cells are fertilized by sperm outside the woman's womb. IVF is a major treatment in infertility when other methods of achieving conception have failed.
The process involves hormonally controlling the ovulatory process, removing ova (eggs) from the woman's ovaries and letting sperm fertilise them in a fluid medium. The fertilised egg (zygote) is then transferred to the patient's uterus with the intent to establish a successful pregnancy. The term in vitro, from the Latin, is used, because early biological experiments involving cultivation of tissues outside the living organism from which they came, were carried out in glass containers such as beakers, test tubes, or petri dishes. Today, the term in vitro is used to refer to any biological procedure that is performed outside the organism it would normally be occurring in, to distinguish it from an in vivo procedure, where the tissue remains inside the living organism within which it is normally found. A colloquial term for babies conceived as the result of IVF, test tube babies, refers to the tube-shaped containeners of glass or plastic resin, called test tubes, that are commonly used in chemistry labs and biology labs. However in vitro fertilization is usually performed in the shallower containers called petri dishes, which may be made of glass or plastic resins (contemporary test tubes may also be made of glass or plastic resin).





A maximum of four pre-embryos will be transferred to the uterus for possible implantation. Patients will have several other options regarding the disposition of the remaining pre-embryos. One option is to freeze pre-embryos for your later use. Other options are to donate or simply dispose of them. Excess pre-embryos, if any, belong to you, and you will determine what is to be done. Fertilization, in vitro: IVF, a laboratory procedure in which sperm are placed with an unfertilized egg in a Petri dish to achieve fertilization. The embryo is then transferred into the uterus to begin a pregnancy or cryopreserved (frozen) for future use. IVF was originally devised to permit women with damaged or absent Fallopian tubes to have a baby. Normally a mature egg is released from the ovary (ovulated), then enters the Fallopian tube, and waits in the neck of the tube for a sperm to fertilize it. The basic steps in an IVF treatment cycle are ovulation enhancement (stimulating the development of more than one egg in a cycle), egg harvest, fertilization, embryo culture, and embryo transfer. Several distinct ethical objections have been made to the use of IVF. Initially, there was concern about the risk that the children born as a result of this procedure would be abnormal. On the other hand, objections on the basis of the cost of the procedure remain serious, especially where the resources are drawn from a limited national health budget. Because the rate of births per cycle of treatment remains low, generally around 15 per cent, the cost of each child produced is considerable. In addition, there is a human cost for those couples whose hopes of overcoming infertility are raised by reading headlines about IVF, but find that they do not achieve a pregnancy. Many reasonably ask if adoption, including overseas adoption, would not be a better solution to the needs of infertile couples. The eggs must first be fertilized in our laboratory, and the resulting embryos then are placed into the uterus 2 to 5 days later. This procedure achieves remarkable pregnancies even in women with hopelessly damaged fallopian tubes and even "unexplained" infertility. When there is any problem at all regarding the sperm, we can fertilize the eggs with ICSI. In fact, in our program we routinely use ICSI in all cases to guarantee against the risk of failed fertilization. Our IVF pregnancy rate is over 50% per attempt, regardless of diagnosis, and we accept all of the most difficult cases.
